The following are forms new and current patients may need. if you are coming in as a new patient you will need to fill out a New Patient Form either prior to being seen either at home or in our office. We know that selecting an appropriate dentist can be a difficult decision and making the transition to becoming a patient involves a few steps. Therefore our office has created this page to provide useful information for you as well as to be a resource to help you get started on the right foot.

 

What kind of dental treatment does this office offer?

Our office offers a wide variety of services for both adults and children. Dr. Zamani can perform endodontic therapy, scaling and root planing, orthodontic treatment, restorative services, crowns, bridges, dentures and implants for missing teeth. For children, we also provide fluoride treatment and sealants on molar teeth to keep teeth strong and prevent cavities.


What steps can I take before I come into the office?

To make your first visit go smoother you can do a few things before you even walk in the door. To start you can visit our 'Download Forms' page and print out a New Patient Form. Fill it out before your first visit and bring it with you. We ask that you bring proof of insurance with you, even if we have taken your information over the phone.

If you have had any major surgery recently or you are on a number of medications you may want to print a Medical History Report and have your regular doctor fill it out and send it to us. For your protection please bring a list of any medications you are currently taking. Additionally if you have been to another dentist within the past year you can contact them to have your dental records and x-rays sent here. If they require a release form to be signed you can download a Request For Dental Records form from the same page.

 

Driving Directions

Our office is located in Towson, MD off of York Rd in the Campus of St. Joseph's Hospital. The address of our building is 120 Sister Pierre Drive suite 503 Towson, MD 21204.

 

From Downtown

Take I-83 North (Jones Falls Express-way) onto Northern Parkway. Follow to York Rd. Turn left and follow York until you reach Sister Pierre Drive. Turn left onto Sister Pierre Drive and then right at the Professional Center sign.

From I-695 East

Travel on I-695 East to exit #26A (Towson/York Rd). Make a left onto West Road and shortly after make a right onto York Road. Follow York Road south past the circle until you reach Sister Pierre Drive. Turn right onto Sister Pierre Drive and then right at the Professional Center sign.

From I-695 West

Travel on I-695 East to exit #27A (Dulaney Valley Road Rd) going toward Towson. Bear left onto York Road. Follow York Road South until you see Sister Pierre Drive on the right. Turn right onto Sister Pierre Drive and then right at the Professional Center sign.
