Appointments
We know you have many choices when choosing a Dentist in Towson, MD so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing
We accept most traditional insurance plans including Medicare and Healthy Smiles. Contact our office to verify acceptance of your plan. We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.

At our office we take pride in giving our patients professional care and friendly service. Dr. Zamani performs a wide variety of dental services including endodontic therapy, scaling and root planing, orthodontic treatment, restorative services, crowns, bridges, dentures and implants for missing teeth. At our office we try to accommodate the schedules of our patients as best we can. Our office hours are 8 AM to 4 PM Monday, Wednesday, Thursday and Friday. We are not open Tuesdays as Dr. Zamani volunteers his time by teaching at the University of Maryland. Saturday mornings we are open by request only and these days are scheduled only after initial consultation. Our office is closed on major holidays, so please check with us to see if we are open.

Patient Comfort

Not every dental procedure can be 100% painless, but Dr. Zamani and the staff at Advanced Dental Care employ the most recent methods of dispensing anesthesia and other advances in dentistry to make your visit as comfortable as possible.

Below are just some of the methods we can use to reduce discomfort, and sometimes eliminate it all together!

Pre-medication

When needed, Dr. Zamani selects medication to mildly sedate or calm patients before and during the appointment. Sometimes this is used to simply help patients sleep the night before an appointment.

Laughing Gas

N2O2 or "laughing gas" as it is sometimes called, offers an excellent alternative for many people without high risk and high cost associated with being put to sleep.

Microdentistry

The days of painful scraping and drilling are nearly over. The latest micropreparation systems allow us to perform most routine dental procedures without the need for shots, the discomfort of numb lips, or the tedious waiting times associated with dentistry in the past. That's what I call a balance between technology and comfort... and it costs no more than doing it the old way.

Infection Control

We are all very concerned about your safety. Every member of our staff is trained in the most recent infection control technologies. To list all the new technologies we use for this would take another whole website!

Fiberoptics

Sometimes, the older technology is the only route available. We have taken effective, older instruments, and made them better. All of our high speed drills are accurately guided by fiberoptic technology for a lower margin of error, and less discomfort for all.

One Time Crown Payment Plan

OUR PART

* If our crowns, porcelain veneers, or onlays EVER need replacement, and the tooth is still saveable... as long as Dr. Zamani maintains his private practice in dentistry... and you do your part ( below ) we will at no charge:

  • Replace any crown with a new crown of the same type
  • Replace any porcelain veneer with a new veneer
  • Replace any only with a new onlay

Note: If a veneered or onlayed tooth is damaged or decayed to the extent that a crown is necessary, the full fee originally paid for the veneer or onlay will be applied toward the crown. You will pay only the difference. We can not predict if and/or when gum or nerve treatments may be required. If needed, they would require additional fees.

YOUR PART

Visit our office for a minimum of 2 ( 4 if we've recommended due to gum condition ) times per calendar year for cleanings. Note: Many of our patients still return to us after moving across the United States. If you move out of the Baltimore Metro area, yet have done the rest of your part ( below ), one cleaning & check-up in our office annually will be sufficient to maintain this agreement!

Complete ( in our office ) all recommended treatment in the quadrant ( quarter of the mouth the tooth in question is in ), all within a one year period. Gum infection and bacterial decay spread from tooth to tooth. Your path to optimal health requires a comprehensive approach to care.

Advise us of the need to change or cancel appointments at least 48 hours in advance. Shorter notice cancellations or changes severely limit our ability to offer another patient their most preferred time.

 

Office Conduct

To respect other patients and office staff we ask that all patients act in an orderly manner when in our office. Please refrain from the use of profanity. We also ask that you not eat or drink in the office to keep the waiting room and operatories clean.

Appointment Cancellations

If you are unable to keep your appointment, it must be rescheduled or canceled at least 24 hours prior to treatment. Any cancellations after this time may be subject to a $50 fee. Advanced Dental Care reserves the right to reschedule your appointment if you arrive 15 minutes or more late for your scheduled time.

Emergency Appointments

In the event of a dental emergency our office will do our best to accommodate your needs with your schedule so you can be seen as soon as possible. We will try to see you the same day, though we cannot guarantee same day service if your schedule is not flexible or you call later than 12 PM. Emergency appointments will not be made on Saturday mornings. Emergency appointments for new patients must be paid in full at the time of service.

Please note that emergency service will be given by dental need and as such will be given based on necessity. These emergencies will be evaluated at the time of call. Dental emergencies that justify same day service are as such: severe dental pain, excessive bleeding, jagged or broken teeth, broken partial dentures or prosthetics, broken or loose crowns and bridges, swollen gums or cheeks or abscesses of the mouth.

Personal Information Changes

Please notify the secretary of any changes to address, phone number, insurance carrier, etc before being seen for treatment.

Insurance Coverage

To minimize your wait before you are seen please inform the secretary of any insurance coverage before you are seen. We accept a variety of PPO insurance plans. If you have any question of whether we accept your plan or not please ask before arriving at the office. To ensure the correct insurance billing, we would ask that for your first visit you bring your insurance card or a statement of proof of insurance that includes your name, carrier, ID number, insurance phone number and address. After we have obtained a copy you do not need to show your card again unless your coverage or carrier changes. If this is to occur please inform our office before you are seen to prevent any billing mistakes.

Non-participating Insurance Holders and Self-Pay Patients

For those patients whose insurance plan we do not accept or who do not have insurance, payment is due at time of treatment unless otherwise agreed upon. For longer treatment plans, such as ongoing orthodontics, our office will be happy to set a payment plan for you. You will be asked to make a down payment of 25% of the total treatment plan on the first visit that treatment is started. The remaining balance must be paid in full before treatment is completed. This service is only provided for those patients who have been seen in our office and evaluated for treatment.

Notice of Privacy Practices

Understanding Your Health Records

Every time you visit a hospital or health care provider your visit is recorded. Usually this record contains symptoms, examination, test results, diagnosis, treatment and/or future treatment plans. These records serve as a basis for planning your care and communicating among the health care providers that contribute to your care. Understanding what is in the record and how your health information is used helps to ensure its accuracy, better understand who, what, when, where, and why others may access your health information and make more informed decisions when authorizing disclosure to others.

Your Medical Record Rights

Although your health record is the physical property of the facility that compiled it, the information belongs to you. You have the right to request a restriction on certain uses and disclosures of your information. This includes the right to obtain a paper copy of the notice of information practices upon request, inspect and obtain a copy of your health record, obtain an accounting of disclosures of your health information, request communications of your health information by alternative means or alternative locations, revoke your consent to use or disclose health information, except to the extent that action has already been taken.

Our Responsibilities

Advanced Dental Care is required to maintain the privacy of your health information, provide you with a notice as to our legal duties and privacy practices with respect to information we collect and maintain about you, abide by the terms of this notice, notify you if we are unable to agree to a requested restriction, accommodate reasonable requests you may have to communicate health information by alternative means, or at alternative locations. We reserve the right to change our practices and to make new provisions effective for all projected health information we maintain. Should our information practices change we will post a notice in the waiting room informing patients that a change has occurred. A copy of the new practice changes can be obtained by request.

How We May Use and Disclose Your Medical Records

Although we are no longer required to obtain a written consent for use and disclosure of protected health information for treatment, payment and health care operations, we still need to have your consent for other special situations listed below. Therefore we request that you sign a consent so that we have the ability to provide complete health care without restrictions. You have the right to request certain restrictions in writing and we can either comply with or deny your request. It is our goal to keep communication clear and open between practice and patients and allow you to know how thus practice functions relative to patient care.

Special Situations

To avert serious threat to health or safety- We may disclose health information about you when necessary to prevent a serious threat to your health and safety or the health and safety of the public or another person.

Emergency treatment- We are allowed to treat emergency individuals in emergency situation, even when the patient does not consent to use and disclosure of health information

Treatment of incapacitated adults- The laws allow information to be disclosed to the legally designated representative of an incapacitated individual, and for granting of consent by such representatives.

Required by law- We will use or disclose health information about you when required to in following the more stringent of State, Federal or local laws

Workers’ compensation- We mat release health records about you for workers’ compensation or similar programs. These programs provide benefits for work related injuries or illnesses.

Health oversight activities- We may disclose health information to a health oversight agency for audits, investigations, inspections or licensing purposes. These disclosures may be necessary for certain state or federal agencies to monitor the health care system, government programs and compliance with civil rights laws.

Lawsuits and disputes- If you are involved in a lawsuit o a dispute we may disclose health information to a court or administrative order. Subject to all applicable legal requirements, we may disclose health information about you in response to a subpoena.

Law enforcement- We may release health information if asked to do so by a law enforcement official in response to a court order, subpoena, warrant, summons or similar process, subject to all legal requirements. Information is not personally identifiable. We may use or disclose health information about you in a way that does not personally identify or reveal who you are.

Family and Friends- We may disclose health information about you to your family members or friends if we obtain your verbal agreement to do so or if we give you an opportunity to object to such a disclosure and you raise no objection. We may also disclose health information to your friends and family if we can infer from the circumstances, based on professional judgment, that you would not object.

Other Uses and Disclosures of Health Information

We will not use or disclose your health information for any other purpose than those identified in the previous sections without your specific authorization. If you give us specific separate authorization to use or disclose health information, you may revoke that authorization in writing at any time. If you revoke authorization we will no longer use or disclose information about you, but we cannot take back any uses or disclosures already made with permission.

Any requested restrictions to your patient health information should be made at the time that you sign the consent or within 30 days thereafter. We are not required to agree to your request for restrictions. If we do agree this will be placed in writing and put into effect unless the information is needed for emergency treatment. To request any restrictions, you may complete the Request for Restrictions form and return it to our office.

If we have HIV or substance abuse information about you, we cannot release that information without special signed and written authorization. In order to disclose those types of records we will have to have both your signed consent and a special written authorization that complies with the law governing HIV or substance abuse records.

A consultation exam will be given free of charge to evaluate your dental needs. Any additional services will be charged at our standard fees. Please contact the office for a price estimate. Exact price quotes can not be given over the phone, only when evaluated in person.

Also for your convenience our office now accepts CareCredit, a third party billing company. We do ask that if you plan on using your CareCredit card that you inform us prior to treatment. Outstanding Balances

Any balance that is 90 days or more past due, unless under prior agreement with our office, will be sent to an outside collection agency. Our office reserves the right to add a $25 finance charge to cases that are sent to collections and may incur an addition charge of up to the amount of the full billing amount depending upon the method of collection. Additionally any canceled or rejected check will incur a service charge or $25 to the account.
